Overview
Social Worker (Medical Social Worker) – Portland, OR. Travel assignment starting 04/20/2026 for 13 weeks in a home healthcare – hospice setting. 40 hours per week, day shifts (5x8). Weekly pay: 2,166–2,283 per week. Guaranteed hours: 0.0.

Responsibilities
- Provide clinical psychosocial services to patients and their families across the lifespan (neonate to geriatric)
- Assess psychosocial needs and coordinate referrals to facilities or community resources
- Deliver crisis intervention to patients and families as needed
- Provide bereavement counseling and support
- Document interventions and care plans in the patient record
- Collaborate with an interdisciplinary team to support discharge planning and placement in appropriate settings

Requirements
- Master of Social Work (MSW) degree
- Ability to provide clinical psychosocial services across all ages in a home healthcare – hospice setting
- Licensure as required by the state for MSW practice (eligible or appropriate licensure per Oregon requirements)
- Experience or demonstrated ability in crisis intervention and bereavement counseling
- Excellent communication and documentation skills
- Willingness to participate in a travel-based assignment and work in patient homes as needed
