Job Description:

This position will assist with pharmacy care management services to ensure the medication needs are met in the in the most safe, efficacious and cost-effective manner for our Accountable Care Organization (ACO) contract enrollees including work within quality STAR and HEDIS measures. This position will also participate in clinical patient education and the completion of accurate patient medical histories as well as engage in identifying additional opportunities to improve patient care and offer suggestions to improve the performance of the pharmacy department. In addition, this position may also assist timely dispensing of medications to patients ensuring accurate labeling and in compliance with all legal requirements, regulatory agency standards, third party contracts and Essentia Health Policy and Procedures.Flexible work hours.

Education Qualifications:

Currently enrolled in an ACPE accredited Pharmacy program.

Licensure/Certification Qualifications:

• Basic Cardiac Life Support (BCLS) certified, or able to obtain within 3 months (BCLS not required for Interns hired into the following departments: 3012410, 4002410, 2002410, 2042410)

• ND: Must be licensed as a Pharmacy Intern through the ND Board of Pharmacy

• MN: Must be licensed as a Pharmacy Intern through the MN Board of Pharmacy

• WI: Completion of 2nd year of a Pharmacy program. No registration required
